During the on-site audit of the Office of Human Resources and Safe Environment, our office was asked by the consulting firm hired by the USCCB to collect a compliance document from each location each year.
Please fill out the attached document and have your pastor sign it. Electronic signatures are not accepted.
The compliance document must be returned to the Office of Child and Youth Protection by Tuesday, June 30.
